We are searching for a casual Case Manager for our Case Management department.
Position Summary:
Ability to function in the capacity of a coordinator, patient advocate, educator, risk manager, facilitator, negotiator, collaborator, consultant, and evaluator based on the needs and values of the patient and in collaboration with all service providers. Assumes responsibility for the coordination of patient centered activities throughout the continuum of health and human services and care settings for a designated cluster of patients during hospitalization. This is to be accomplished in collaboration with a multidisciplinary team while ensuring that the care provided is safe, effective, efficient, client-centered, timely and equitable. Performs work process, which includes securing authorization/certification for inpatient services; accomplishes positive patient outcomes in accordance with quality monitoring including patient care protocols, evidence-based standards of care, and patient and family education. The working framework of the Case Manager consists of interaction, assessment, planning, implementation, and finally evaluation. Desired outcomes are continually monitored, reassessed, and revised, if needed to improve patient services.
Required Education/Experience:
1. Current Michigan licensure as Registered Nurse required.
2. BSN or must be actively pursuing Baccalaureate degree, to be completed within three years of accepting position.
3. Three years of acute care nursing experince with two years CCU or ICU experience if staffing for a critical care position.
4. Flexibility of hours/days to meet needs of patients, families and organization.
Preferred Education/Experience:
1. National certification.
2. Nursing skill mix equivalent to setting.
3. Utilization review experience.
